CLIENTS

The New Client Packet includes all forms needed for starting the therapy process.  

This includes the Informed Consent form (explains my practice and all office policies), the Client Intake form (a questionnaire), the Credit Card Authorization form, the Release and Liability Waiver (for sessions outside of the office), and Video/Phone-Based Therapy Informed Consent Form (for sessions through phone or video).
